This is an urgent reminder regarding the Medical Check-up Circular and Consent Slip uploaded on the ERP yesterday.

All parents are requested to carefully fill in and sign the consent slip and send it to school with their child tomorrow, 2 September 2026, without fail.

Please note that the MAX Hospital medical team will be available at the school for a limited period, and the duly completed consent slip is essential for your ward to avail the medical check-up facility.

We request you to kindly complete the form today itself and place it in your child’s school bag to ensure that it is not inadvertently missed tomorrow morning.

Your prompt cooperation in ensuring 100% submission of consent slips will be highly appreciated.
